La segmentación de mercado constituye una de las herramientas principales para conocer el mercado de anticonceptivos, visualizar dónde se están focalizando los recursos e identificar problemas de acceso y equidad entre quintiles socio-económicos. Este análisis muestra que los esfuerzos por expandir el acceso y el uso de planificación familiar en Honduras han sido exitosos, pero que aún existen discrepancias en las tasas de fecundidad global y en la tasa de prevalencia anticonceptiva entre las poblaciones urbanas y rurales y entre los grupos socio-económicos en Honduras y que la necesidad insatisfecha alcanza un 16.9 por ciento. This assessment of laboratory services and logistics system follows an initial technical assistance visit by the DELIVER project, which was conducted in June–July 2005 to map the Ghanaian laboratory quality assurance referral system. The current assessment of the laboratory system conducted by the DELIVER project (headquarters and Ghana field office) and the Ghana national counterparts in March 2006 is a first step toward the design of a laboratory logistics management information system.

For some time, Ghana managed and distributed public-sector health commodities through the operation of four vertical supply chains, which were judged by the Ministry of Health (MOH) to be ineffective. In its medium-term strategy, which ended in 2001, the MOH noted the need to improve the efficiency of health commodity supply chains to address the nation’s public health goals. The USAID | DELIVER PROJECT has since then proposed and implemented numerous supply chain improvements (especially in procurement management, storage, and distribution) with the aim of reversing the observation of the Ghanaian health authorities and fulfilling the USAID strategic objective of improved health status.

The USAID | DELIVER PROJECT carried out by JSI in Honduras benefited in particular the Ministry of Health (MOH) and the Honduran Family Planning Association (ASHONPLAFA). The project’s main accomplishments were the development of local capacity to plan procurement that uses logistics information from family planning activities, the development of software for managing inventories and logistics management information for the MOH, and the formation and consolidation of the Contraceptive Security Committee and the contraceptive security (CS) national strategy, which involved a variety of national stakeholders and was ratified by the MOH.

In India, the USAID | DELIVER PROJECT worked in three states, namely Uttar Pradesh (UP), Uttaranchal, and Jharkhand, in northern India. However, many of the technical assistance activities during this report period were focused on UP and then on Uttaranchal. When the project started working in UP, the largest state in India with 160 million people, contraceptive prevalence rate was barely 19 percent, and the entire public sector contraceptive logistics system was in disarray. Therefore, many of the interventions involved setting up systems, system streamlining, training, and system strengthening in working with local partners, such as the State Innovation in Family Planning Services Agency (SIFPSA) and the Indian Institute of Management, Lucknow. By 2005, the contraceptive prevalence rate in UP had risen to 29 percent.

The DELIVER Project in Kenya began its operations in October 2000 after the Family Planning Logistics Management III Project ended. The primary goal of the project at that time was improved availability of contraceptives and other essential health commodities at service delivery points. The main strategy toward this end was to strengthen health sector logistics management systems, and the main partner was the Ministry of Health Division of Reproductive Health. Over the next five years, the USAID | DELIVER PROJECT saw an extraordinary increase in its scope of work. In 2006, the Logistics Indicator Assessment Tool (LIAT) was used to take stock of the performance of the logistic systems and to provide stock status information for key public health commodities. Results of the assessment revealed that nearly all systems were implemented as designed, but strengths and weaknesses continue to exist. The widespread availability of key commodities for each program was found throughout the logistic systems. In addition, both commodity availability and logistics management practices in the ARV program are performing well.

An assessment of the contraceptive security (CS) situation in Liberia was carried out by a team of DELIVER consultants working with the Family Health Division of the Ministry of Health and Social Welfare (MOHSW.) Using an adapted SPARHCS (Strategic Pathway towards Reproductive Health Commodity Security) framework and tool, the team reviewed documents, met with stakeholders and visited health facilities for the public, nongovernmental and commercial sectors in five of the fifteen counties in Liberia. This assessment will serve as the basis for the development of a national CS strategy.

In March 2006, DELIVER conducted a nationally representative survey on the availability of selected health commodities at Ministry of Health (MOH) and nongovernmental organization (NGO) facilities throughout Malawi. The information provided through this survey, along with the results from any previous surveys, will allow the MOH and its partners to monitor the effectiveness and efficiency of the health commodity logistics system and to make adjustments as necessary. The Logistics Indicators Assessment Tool (LIAT), which was tailored in Malawi in 2004, was used again with minor adaptations to address current assessment needs. Comparative analysis was conducted with the results from the 2004 survey and to determine any change in the health commodity logistics system over the last two years.

Over the last four years, the DELIVER Project in Mali assisted the Ministry of Health (MOH) and its stakeholders to carry out appropriate activities to improve the program. Throughout the life of the project, supervisory visits were conducted by a team composed of project’s resident advisor, a representative of the Direction de la Pharmacie et du Médicament, and a regional pharmacist. Health care management is a challenge in Nepal because of geographical adversities, resource constraints, and the recent conflict situation. The goal of DELIVER’s technical assistance to the Ministry of Health and Population’s health logistics activities is to improve the availability of contraceptives and other key program commodities at the health facility and community level, thereby increasing service utilization. Availability of key commodities at the service delivery points, increased reporting percentages, and improved storage standards at all levels of the health institutions are key to delivering essential health care services to the Nepalese people. DELIVER support focuses on capacity building and skills transfer to the government system for long-term sustainability of the program.

Regulatory guidelines under the Nepal Drug Act of 1978 govern the sale and distribution of pharmaceutical products in Nepal. Regulations are based on a 1995 National Drug Policy that promotes community drug financing programs in the public sector and establishes profit and distribution margin rates. Those policies were put in place to encourage equity, access, and use of medicines. The findings from this analysis indicate that cumulative distribution and profit margins for some reproductive health (RH) medicines in the private sector exceed the established norms.

DELIVER’s technical support has significantly improved the Ministry of Health’s (MOH) contraceptive availability in all the service delivery points. This successful experience in family planning logistics has also improved the process of integration for the other essential drugs. In June 2006, the Information System for the Integrated Logistics System for Essential Drugs and Contraceptives (SIGLIM) was implemented in five regions with the expectation of being extended to the rest of the country in 2007. DELIVER has also strengthened the postpartum family planning services in 22 Mother and Child Health Care (MCHC) hospitals.

In response to growing recognition of the magnitude and devastating effect of the HIV/AIDS pandemic in Tanzania and the increasing commitment from government and international donor organizations to support HIV/AIDS Prevention and Care, USAID/Tanzania asked the John Snow, Inc/DELIVER Project to collaborate with the Ministry of Health in conducting an initial assessment of 1) the key commodities and supplies needed to support HIV/AIDS prevention and care activities in Tanzania, and 2) the capacity of the existing logistics systems to monitor and distribute these commodities.

This case study presents the story of the USAID-funded Romanian Family Health Initiative (RFHI) from 2001 to 2005, and how it succeeded in expanding access to and use of family planning services and supplies. It presents both the achievements of the program as well as lessons learned during the preceding 1999–2001 pilot program and during the scale up phases of the program. The study is intended primarily for program managers, policymakers, donors, and providers of technical assistance in Eastern Europe and Eurasia.
